Abstract

Background: Xeroderma pigmentosum (XP) is an autosomal recessive genetic disorder characterized by photosensitivity, cutaneous pigmentary changes, and malignant tumor development at an early age. The basic defect underlying the clinical manifestations is nucleotide excision repair defect, leading to defective repair of ultraviolet (UV)-induced DNA. XP patients who are younger than 20 years of age have more than 1000-fold increased risk of developing malignant neoplasms of the skin, which commonly include squamous cell carcinoma, basal cell carcinoma, fibrosarcoma, and malignant melanoma. Malignant melanoma arises in only about 3% of XP patients. Purpose: To report a case of malignant melanoma in a child with XP. Case: A 7-years-old girl presented with multiple hypopigmentation and hyperpigmentation macules since age of two, throughout the body, more on sun-exposed areas. The physical examination showed solitary tumor extensive ulcero- proliferative surface with areas of hemorrhage and blackish pigmentation on the vertex region. Histological examination revealed a feature of nodular malignant melanoma, and the condition became worse after she underwent two cycles of chemotherapy. Discussion: Despite the rare occurrence, the nodular type of malignant melanoma in XP patients is the most aggressive and responsible for the fatal condition. Conclusion: Early detection of XP is necessary due to its fast-growing nature and high metastatic possibility as well as mortality index.

Abstract

Background: It is hypothesized that combination of amniotic membrane stem cell metabolite product (AMSC-MP) and vitamin E after fractional CO2 laser as laser assisted drug delivery (LADD) provides better long-term effects on clinical improvement of photoaging. This is promising as an option for photoaging therapy in the future. Purpose: The study aimed to evaluate long-term effects of improving the degree of wrinkles, polarized black spots, ultraviolet (UV) black spots, pores, and skin tones in subjects who have received a topical mixture of AMSC-MP with vitamin E and CO2 fractional laser as LADD. Methods: Thirty adult women with photoaging who had been treated with topical AMSC-MP mixed therapy with vitamin E and CO2 fractional laser three times, were then given maintenance therapy, tretinoin cream 0.025% and sunscreen SPF 30. Observation of improvement of wrinkles, polarized black spots, UV black spots, pores, and skin tones were performed computer-stimulated photographs, Janus-II Facial Skin Scope System, three times every month for three months after the last treatment. Results: The evaluation of wrinkles, polarized black spots, UV dark spots, pores and skin tones from the assessment for three months after being receiving treatment of a topical mixture of AMSC-MP with vitamin E and CO2 fractional laser as LADD on photoaging obtained p>0.05. Conclusion: There was a long-term effect of the topical mixture of AMSC-MP with vitamin C and CO2 fractional laser on photoaging, and there was no side effect observed.

Abstract

Background: Malassezia folliculitis is a pilosebaceous follicular infection disease caused by Malassezia species. There are many misdiagnosed Malassezia folliculitis cases, causing the maladministration of therapy. A routine diagnostic test performed for Malassezia folliculitis cases is the identification of fungal elements (spore) with a microscope using potassium hydroxide, but it has several weaknesses. Purpose: To evaluate the suitability of Malassezia folliculitis diagnostic test using Potassium Hydroxide 20% + Blue-Black Parker Ink, May Grunwald Giemsa, and Potassium Hydroxide 10% + Chicago Sky Blue. Methods: Analytic observational study conducted in the Dermatomycology Division of Dermatology and Venereology outpatient clinic, Dr. Soetomo General Hospital Surabaya. The samples were thirty patients with clinical features of Malassezia folliculitis. The research material was obtained from the body as many as three pieces of papulomoluscoid lesion extracted. The material obtained was then divided into three glass objects for Potassium Hydroxide 20% + Blue-Black Parker Ink, May Grunwald Giemsa, and Potassium Hydroxide 10% + Chicago Sky Blue staining. Result: The identification of spores using Potassium Hydroxide 20% + Blue-Black Parker Ink was 90%, May Grunwald Giemsa was 90%, and Potassium Hydroxide 10% + Chicago Sky Blue was 93% with a value of κ=0.348 and p=0.051. The diagnostic values of May Grunwald Giemsa and Potassium Hydroxide 10% + Chicago Sky Blue were 96.6% sensitivity, 33.3% specificity, 92.9% Positive Predictive Value, and 50 % Negative Predictive Value. Conclusions: There was no significant concordance between May Grunwald Giemsa and Potassium Hydroxide 10% + Chicago Sky Blue with Potassium Hydroxide 20% + Blue-Black Parker Ink in establishing the diagnosis of Malassezia folliculitis. Potassium Hydroxide 20% + Blue-Black Parker Ink is still needed as a routine examination in cases with clinical features of Malassezia folliculitis.

Abstract

Background: Atopic dermatitis (AD) has a significant negative impact on the quality of life (QoL) of affected children and their families. Despite the availability of efficacious treatment, poor knowledge of AD treatment, and adherence to daily controller medications leads to a high rate of treatment failure. Purpose: To characterize the efficacy of educational interventions to improve AD outcomes in the pediatric population. Methods: A systematic search of PubMed, OVID Medline, CINAHL, and PsychINFO was conducted from September 2010 through September 2018. We compared populations, intervention characteristics, study designs, outcomes, settings, and intervention levels across studies. Result: Of 85 articles identified and reviewed, 7 articles met inclusion criteria. All included studies demonstrated efficacy in overall outcomes. All studies were performed in a hospital setting or utilized technology. The most common format for pediatric AD intervention delivery was the use of group lectures. Promising strategies to address time limitations in parents of children with AD include multifaceted educational support programs, individually tailored nurse consultation, and Web-Based Educational Programs. This systematic review is not a meta-analysis, therefore limiting its quantitative assessment of studies. Conclusion: Educational interventions demonstrate efficacy in improving pediatric AD outcomes. As the seven included studies employed a wide variety of outcome assessment tools, it is difficult to compare the extent of improvement in outcomes across these studies. In order to better assess the comparative efficacy of different educational interventions to improve outcomes in children with AD, it is essential to standardize outcome measures.

Abstract

Background: The incidence of drug eruptions is increasing during the last few years due to a large number of new medications. Early detection of the causative agent and the prevention from exposure are crucial managements in terms of drug eruption, mainly to prevent its recurrence. Objective: To understand skin test as a diagnostic modality in drug eruption, which includes skin patch test, skin prick test, and intradermal test. Literature review: Drug eruption is a form of skin eruption triggered by the use of medications, topical or systemic, in the right dose and indication. The manifestation can vary from maculopapular, urticaria, pustular, and bullous eruption; from the most nonsignificant to a life-threatening reaction. A diagnostic procedure is critical to discover the type of drugs that cause the eruption, i.e., skin test, specific IgE measurement, histamine-release test, and provocation test. Skin test is the first choice in the diagnostic process as it is simple, easy, practical and has high sensitivity and specificity. Conclusion: Skin test is one of the many available diagnostic tools.  However, both false positive and false negative results might still arise. The experts are currently attempting to come up with more accurate and practical tests to aid the diagnostic of drug eruption, thus preventing its occurrence.

Abstract

Background: Photoaging causes wrinkles, lentigines, keratoses, dyspigmentation, telangiectasia, decreased elasticity, rough texture, and pale color. Facial wrinkles and mottled skin can negatively affect perceived attractiveness, self-esteem, personality, and level of acceptance. Stem cells are important cells in the skin, and they are the source for continuous regeneration of the epidermis. It can be extracted from many sources, including amnion. Amnion membrane stem cell (AMSC) can be cultured and secretes metabolite product, and it can rejuvenate aging skin. Laser-Assisted Drug Delivery (LADD) involves selective destruction of the layers of the epidermis and dermis to allow penetration and absorption of topical drugs as well as large molecular weight drugs such as stem cells. Purpose: To evaluate the long-term effects of topical administration of AMSC metabolite products (AMSC -MP) and fractional CO2 laser on the clinical improvement of photoaging compared to previous skin conditions. Methods: This was an observational study which was aimed to evaluate the long-term effects of clinical improvement after topical therapy of AMSC-MP and fractional CO2 laser in patients with photoaging. Results: The results of the analysis showed a decrease in wrinkles, dark spots, skin tone, and pores. Conclusion: Based on Janus-II® assessment, there was improvement degrees of wrinkles, dark spots, and pores in patients with photoaging who received fractional CO2laser fractional for 12 weeks. There were no side effects found in this study.

Abstract

Background: Recently, the incidence of syphilis among men who have sex with men (MSM) has increased rapidly, especially among Human Immunodeficiency Virus (HIV)-infected MSM. Coinfection with these two organisms alter the symptoms and signs, progression of the disease, and the risk of progressing to the tertiary stage. Purpose: To report a case of secondary syphilis in HIV-infected MSM.  Case: A 24-year-old male complaint of multiple redness macule for 1-month duration on his chest, back, groin, palm, and soles. The rash was neither painful nor pruritic. He also complains of hair loss resulting "moth eaten" alopecia. One month before, he had a painless ulcer on his genital, which resolved without treatment. His HIV infection was diagnosed two years earlier. He had been sexually active with multiple homosexual partners without using a condom since 2010. The Venereal Disease Research Laboratory test (VDRL) titer was 1/32, and Treponema pallidum haemagglutination assay (TPHA) was 1/20480. Initial treatment was a single dose of 2.4 million units of benzathine penicillin. Serologic examination was reevaluated on month 1, 3, 6, and 9 after therapy and declined in the third month. Discussion: For HIV-infected persons, the clinical manifestations of syphilis in most of the cases remain the same. However, the lesions are more aggressive, and coexistence of primary and secondary syphilis is more frequent. Serologic tests are accurate and reliable for the diagnosis and for following a patient’s response to treatment. Penicillin is effective but physical and serological follow up is needed. Conclusion: HIV-infected MSM have higher risk of syphilis. Staging is needed to determine the treatment. Serologic examination should be repeated and long enough to monitor the treatment success.

Abstract

Background: Acne vulgaris (AV) is a chronic inflammatory disease of the pilosebaceous unit, particularly among adolescents. The pathogenesis of AV is multifactorial, developing research studies  the role of free radicals and antioxidants imbalance that cause oxidative stress in AV. The main antioxidant found in the skin is vitamin E, which functions as a protector against lipid peroxide. Purpose: To compare serum vitamin E levels in adolescents with AV and healthy adolescents without AV (controls). Methods: This is a cross-sectional observational analytic study that involved 17 adolescents with AV and 17 controls in Dermatology and Venereology Outpatient Clinic Dr. Soetomo General Academic Hospital Surabaya. The subjects have met the inclusion and exclusion criterias. Result: The mean of vitamin E level in adolescent patients with AV was 7.8 ± 1.07 mg/mL and 10 ± 1.06 mg/mL in controls with the p-value in this study was p = 0.0001. Conclusion:  It was found that serum vitamin E levels in adolescent AV patients were significantly lower than the controls. Further research is required to find out more about the role of antioxidants in the pathogenesis of AV.

Abstract

Background: Vitiligo is an acquired and commonly found pigmentation disorder characterized by milky-white patches on the skin, hair, and mucosa due to melanocyte damage. The cause of vitiligo is still unclear. A study proves that cell-mediated immunity plays a role in the pathogenesis of vitiligo. Melan-A is a melanoma-related antigen that is recognized by autologous cytotoxic T cells and one of the critical markers for detecting melanocytes. Objective: To evaluate the expression of Melan-A in depigmented lesions of vitiligo patients. Methods: A descriptive study aimed to describe the expression of Melan-A in the depigmented skin of vitiligo patients at the Dermatovenerology Outpatient Clinic Cosmetic Division of Academic General Hospital Dr. Soetomo Surabaya. Eleven study subjects were selected through a sequence of selection. Results: Melan-A expression in the depigmented skin of vitiligo patients was lower than the average. This result was found in 6 (54.55%) out of 11 patients. Conclusion: Melan-A expressions on depigmented skins of vitiligo patients are generally below the average value; therefore, adequate intervention is needed to increase the Melan-A expression.

Abstract

Background: Infantile Hemangioma (IH) is a benign vascular neoplasm and most commonly found in newborns. IH appears in the first few weeks of life and mostly resolve immediately. Early recognition of symptoms, diagnosis, and therapy has an important role in determining the prognosis of this condition. Purpose: The purpose of this study is to evaluate the clinical features of new IH patients in the Pediatric Dermatology Division of the Dermatology & Venereology Department outpatient clinic of Dr. Soetomo General Hospital Surabaya in 2008-2017. Methods: This is a retrospective descriptive study from the medical records of new IH patients over a period of 10 years (2008-2017). Results: A total of 84 new IH patients were observed in this study. The data showed that: the most age group was 0-1 year old (71.43%), female (63.1%), and no family history of a similar condition (94.05%). The most common complaint reported was the presence of bumps (59.52%). Most complaints occurred before the patients turned one year old (89.29%) and involved the facial region (58.33%). From the clinical features, the most frequent skin lesions were macules (35.36%). In addition, there was a combination of macular and nodular lesions in 28.13% of patients. The most widely used therapy is laser modality found in 55.45% of patients. Conclusion: Clinical features of IH patients, in accordance with the theory and treatment choices, dependent on the patients’ conditions and clinical manifestations of the disease.
